Participation Forms

In order to participate in SEMHIE efforts, all organizations must complete
a Membership Agreement

Those organizations wishing to participate in a governance capacity will be required to complete a 2nd form, a Conflict of Interest Disclosure Form.

If you are interested in participating in a work group, please contact us and provide us with your completed Membership Agreement.
