Participation FormsIn order to participate in SEMHIE efforts, all organizations must complete 2 forms:Those organizations wishing to participate in a governance capacity - either on the governance work group or governance committee - will be required to complete a 3rd form, a Governance Body Conflict of Interest Disclosure Form. If you are interested in participating in a work group, please contact the appropriate work group chair, as listed below, and provide them with your completed Joint Participation Agreement and Conflict of Interest Disclosure forms. | ||||||||||||||||||||||||||||||||||||||||
